Employment and Income Verifications

Third parties and former employees should direct all inquiries to The Work Number® from Equifax (CMU's employment and income verification service provider). For more information, view the Employee Guide to The Work Number® [pdf].

HR Services handles specific requests from the Department of Defense and the National Security Agency.

Public Service Loan Forgiveness Employment Certification

  1. Complete sections 1–2 of the Public Service Loan Forgiveness Employment Certification Form [pdf].
    1. Do not add your Social Security number to the form at this time.
  2. Email the form to HR Services for completion of sections 3–4 .
  3. An HR Services team member will call you to verify your identity. Identity verification is required before we can return the form to you.
  4. Add your Social Security number to the form.
  5. Send the completed form to the U.S. Department of Education, as indicated in section 7 of the form. Retain a copy for your records.

Sexual Misconduct/Abuse Disclosure to Prospective Employers

Please forward Sexual Misconduct/Abuse Disclosure (Act 168) requests to the staff ombudsperson at act168@andrew.cmu.edu as soon as possible to ensure a response in the required timeframe. Supervisors should not respond to disclosure requests.
